Based on multiple "where IS this policy you speak of?" complaints, I get to propose a policy for physical assessments on our in-pt. hospital psych patients. The theoretical policy is that patients get a full, head-to-toe nursing assessment once a week during day shift unless they have a medical condition that warrants it more frequently.
So, if anyone would be willing to share, what kind of facility do you work in and how often do you do a physical assessment? Or what do you know of ones in your area?

I work in my state's biggest and most acute psychiatric hospital, on an open ward. We do a comprehensive physical assessment on admission, when someone's physical state changes significantly, or more frequently if someone has a medical illness.


Personally, if I were a psychiatric inpatient (which I have been), I would find a weekly comprehensive physical assessment really intrusive.

When asked, my manager thought maybe once a day might be needed! We have patients that get paranoid if you take their blood pressure more than twice a day, a full physical every day would just be too much.

Maybe a mental status exam could be done once a week-- but surely not a physical.

Nurses do their mental status exams, a "are you hearing voices still? Are you feeling depressed, anxious?" twice a day, day and evening shift. Doctors are supposed to do one daily as well, though sometimes it seems like every other day. :P

Daily mental status exams are usually done everyday where I've worked as well-- I guess I just can't wrap my head around doing a head-to-toe so frequently.
